Auto-augmentation recruits expanded and existing breast tissue to an optimal position. It can be thought of as tissue transposition from the lower pole of the breast to the implant pocket, and adds little risk and time to a traditional mastopexy procedure. The tissue is soft, mobile, and looks and feels like breast tissue because it is breast tissue. This is a very effective strategy to provide meaningful bulk and shape to the breast that has excess skin and little native volume. However, when this is not enough, additional strategies such as fat grafting can be used to enhance the result. Fat grafting is generally taken from areas were fat has not desired, such as the abdomen or thighs, processed, and reinjected to form medial or superior pole fullness, enhancing cleavage and breast shape altogether.
