Embryo transfers are now more successful than ever, thanks to improved techniques in the lab that allow for growing embryos to day 5 and beyond and genetic testing to select the embryo with reproductive potential.
Key steps to optimizing embryo transfer outcome:
•Pick the best embryo (based on genetic testing and morphology grading, speed of development)
•Assess the uterine cavity for polyps/fibroids/scar tissue - this can be done with a saline sonogram or a hysterosalpingogram (HSG) or a hysteroscopy (which is a direct view of the uterine cavity) - if any of these problems are found they should be removed and fixed before doing a transfer
•If known history of STIs/ruptured appendix/endometriosis, do HSG to identify swollen fallopian tubes (hydrosalpinx)- if present, the tube(s) should be disconnected from the uterine cavity (surgically removed or cut off from the uterine cavity)
•Thicken the lining either with natural or induced ovulation vs. estrogen in a medicated cycle and move forward when you reach your personal best - ideally 7mm or thicker but this could be thinner and still be successful (in patients who have a tendency toward thin lining without risks for uterine scarring (asherman syndrome).
•Optimizing overall medical health - healthy lifestyle, working towards a healthy weight/BMI, avoiding/quitting smoking, controlling any chronic medical problems, etc.
•Catheter check (which can be done at the same time as the saline sonogram) to confirm I can get to the top of the uterus easily with the embryo transfer catheter. An easy, straightforward transfer always has a better chance of working. Knowing if there will be a sharp turn in the cervix, or a narrowing in the endometrial canal can help to prepare for transfer.
•Do the transfer under ultrasound guidance to ensure the embryo is placed in the right spot.
