This is a simple summary of the way many women take progesterone or progestogen as part of HRT as part of BMS guidelines.
Progestogen is synthetic, there are many different types, like Levonorgestrel.
Progesterone is the natural hormone produced by our body, and is the same as body identical HRT, Utrogestan.
Some women are intolerant to progesterone or progestogen, this can settle with time but if not, it is possible to take progesterone vaginally, and on different schedules to manage this.
Please discuss with your Dr before you make any changes. The metabolite of progesterone, allopregnanlone, acts on the GABA receptor in the brain and this is a bimodal receptor meaning in some women progesterone calms and makes them sleepy, and in other women they have the opposite response with mood changes and insomnia. They are intolerant to progesterone and may have had trouble tolerating any hormonal contraception through the years, and suffered PMS or PMDD in the luteal phase before their period, in an ovulatory cycle, as a response to the rise of their own progesterone after ovulation.
Your annual review is a good time to discuss the right time to change from a cyclical or ‘bleed’ HRT to a continuous or ‘no bleed’ preparation of HRT and to discuss this with your Dr.
