A 69-year-old male farmer from Ohio with no significant medical history presents with right elbow pain of 1 month's duration and firm, affixed, erythematous, and nontender papulonodules of the extremities and trunk after a fall. Some lesions express caseous material. There is no lymphadenopathy.
