It’s #WorldPatientSafetyDay! In the Eastern Mediterranean Region, up to 18% of adverse events are caused by medication errors. Most medication errors are preventable. The #COVID19 pandemic has led to an increased risk of medication errors and unsafe medication practices. WHO’s third global patient safety challenge “Medication without Harm” aims to reduce severe avoidable medication-related errors by 50% globally over the next 5 years. WHO has developed patient safety tools, frameworks, and interventions to address unsafe medication practices. Regional patient safety tools and frameworks address unsafe medication in 3 main areas: 1️⃣Polypharmacy 2️⃣Care transition 3️⃣High risk situations To ensure safe practices, we must comply with the 5 rights of medication use: ☑️Right patient ☑️Right drug ☑️Right time ☑️Right dose ☑️Right route Nobody should be harmed when using medication.
